VENT/TRACHEOSTOMY CARE
VENT/TRACHEOSTOMY CARE RESPONSES

Respond immediately to alarms sounding despite if there is a respiratory team
Perform a respiratory assessment, check stoma and skin, document all findings, report abnormal findings to the MD and Supervisor

Check all orders and compare to what is at bedside to ensure you are compliant with following the current orders, such as vent settings and trach type/size
Provide your staff with a complete report to ensure quality care is being rendered
Know the locations of the Clinical Policies and Procedures and adhere per your scope of practice
Perform hand hygiene and always maintain aseptic techniques
When suctioning, follow the physician’s orders and document amount, color, and consistency
Trach Care
Gather all supplies, wash your hands, remove inner cannula, cleanse cannula
Clean the skin around the tracheostomy ensure you are using aseptic techniques
Understand inner cannula can be re-usable or disposable, never suction without the inner cannula in place

Understand it is your responsibility to assess the patient before and after HD including access site, vs, weight, and mental status
Document all your findings, provide the dialysis center with an accurate report to also include the required follow-up
Notify the physician with any pre or post dialysis orders, labs, and any abnormal findings
Always monitor the patient for signs/symptoms of bleeding and infections
Understand the lack of bruit or thrill and any abnormal findings needs to be reported immediately to the Physician
Ensure to confirm the 5 rights before providing the enteral feeding and document patient’s response
Ensure you complete a GI assessment and check stoma site during your shift and document
If you do not understand how to use the tube feed pump notify your supervisor to receive support to ensure safe delivery of care

Always confirm the 5 rights prior to a medication/treatment pass
Document all yourAlways assess patients for pain and administer pain medication per physician ‘s orders. FYI sleeping is not an indicator of patient not in pain findings, provide the dialysis center with an accurate report to also include the required follow-up
Document the patient’s response to the pain medication if ineffective notify the physician
If medications are not available, make every attempt to get medication call pharmacy and notify the supervisor
Understand treatments are to be completed per physician orders, document all refusal and the education provided
Labs
Understand it is your responsibility to find out if labs were drawn and notify the physician if abnormal
If you do not know how to address labs notify the supervisor for assistance
Commonly used Medications
Coumadin is an anticoagulant it increases the risk for bleeding, lab to consider PT/INR
Administer Insulin as ordered, follow the sliding scale and the frequency of the blood sugar including parameters
When administering psychotropic medications also document non- pharmacological interventions initiated
